The National Primary Health Care Development Agency, NPHCDA, says Nigerians with cases of missing COVID-19 vaccination cards are expected to produce court affidavits and police reports before they can get replacements.
This was contained in a document published by the NPHCDA, and made available to newsmen.
The document revealed that citizens who get vaccinated are given vaccination cards at point of vaccination which serves as proof of vaccination.
It, however, direct citizens who lose their cards to get court affidavits and police reports adding, if the client’s record is found either on the physical record or the EMID portal, a new card will be issued without delay at that same vaccination site.
